AccuBase A1c At-Home Test Kit Return Policy

DTI Laboratories, Inc. (DTIL) AccuBase A1c At-Home Test Kit Return Policy:

Each AccuBase A1c Test Kit is custom manufactured to individual specifications and cannot be returned according to FDA policies without the express written authorization from DTI Laboratories, Inc.

Should DTIL determine that a manufacturing defect is recognized, DTIL will issue a "Return Goods Authorization" (RGA). This registered RGA identifier will be sent directly to the consumer and should be placed in the RGA return mailer.

Manufacturing Defect Process

If you believe your AccuBase A1c Test Kit has a manufacturing defect, please contact our customer service team immediately. Our quality assurance specialists will review your concerns and determine if an RGA is warranted.

Only test kits with confirmed manufacturing defects that receive proper RGA authorization may be returned. This policy ensures compliance with FDA regulations for medical devices and maintains the integrity of our testing process.

Important Notes

• Test kits cannot be returned for reasons other than confirmed manufacturing defects

• All returns must have prior written authorization from DTI Laboratories

• Returns without an RGA number will not be accepted

• This policy is in accordance with FDA regulations for medical diagnostic devices
